• This form is only to be used for Columbia students to include Gamecock Gateway and Palmetto Pathway programs.
• Students may purchase new meal plans by visiting the CarolinaCard Office located on the first floor of the Russell House or the Pendleton Street Garage.
• First-year students living in USC housing are required to purchase an All-Access meal plan for two consecutive semesters. First year students living in USC housing who have a Greek house meal plan have the option to downgrade their All-Access meal plan to the Weekly 10 or Block 100.
• All students have the option to upgrade their meal plan. Upgrades can be made using Self-Service Carolina through the add/drop deadline and directly with the CarolinaCard after the add/drop deadline.
• Students who are suspended from the University by the Office of Student Conduct will not receive a refund for their meal plan.
